List of Documents Required for Fresh/Renew Passport application:

Proof of Date of Birth and Place of Birth	School leaving Certificate Birth Certificate Affidavit – Annexure-A (for illiterate person only)
Proof of Identification	Any one from:- Driving License Election / Voter’s card Aadhar card Bank passbook with photo and duly stamped from Nationalized Bank Ration card with Photo ID. Any State / Central Govt. Issued Identity Card with details of current address proof
Proof of Residence Please Note:	Any one from : – Electricity Bill Water Bill Municipal Tax Bill Telephone Bill Gas connection Bill Ration card with Photo ID Lease agreement Documents must be provided with 1 year consecutive stay of applicant prior to the date of lodgment of Passport Application.
Proof of Education	Std 10th Mark sheet Graduation Certificate
Other requirement if applicable	Marriage certificate
For Govt / PSU employees	No objection certificate from concern Authority
For Dependant of Govt / PSU employees	Annexure – B from relevant authority / Department
For minor applicant	Birth Certificate School Boinafied Student Certificate (if applicable) Valid passports of Both Parents.
For Tatkal Passport	A verification certificate issued by Designated officer on his/ her letterhead with duly signed and stamped of concern officer along with Affidavit Annexure – I.
Other documents	Other relevant documents can be asked by APO / RPO at the time of submission.
